On the obverse are coins in the center of the Small State Emblem of Ukraine, under which the year of coinage is 2005 and circular inscriptions: NATIONAL BANK OF UKRAINE FIVE CHRYVEN (bottom), and also the logo of the Mint of the National Bank of Ukraine.
The reverse of the coin depicts the main street of the city by the Transfiguration Cathedral, against which the emblem of the city of Sumy and the semicircle contain the inscriptions: SUMY (top), 350 years old (bottom).
